Coroner rules teacher's death from blood clot was avoidable

A coroner found that a primary school teacher from Eltham would have survived if a blood clot had been identified and treated. The inquest identified clinical judgment errors in her care at a GP surgery and Queen Elizabeth Hospital, but no neglect. The NHS trust will review the findings.
